If your oral surgeon won't schedule your surgery in a hospital, get another doc. Seriously. Don't mess around.

I feel your pain on the not feeling good part. With dialysis, the body doesn't make enough red blood cells, so my hubby is slowed down considerably. Unfortunately, he can't get the med to produce more blood cells until he is severely deficient and then they will only give him enough to be mildly deficient, not enough to fully raise him back to normal. Rational seems to be similar as you are seeing with the cortisol (though different side effects, of course). Gotta love it.
